Required before your first lesson

Personal details, licence information, emergency contact, your selected courses/services and the terms of instruction — signed digitally.

Have these ready:

  • Your Class 7 (or higher) Alberta licence — number, class, issue & expiry dates
  • Your home address and a mobile phone number
  • An emergency contact's name and phone number
  • If you're under 18: your parent or guardian's name and signature
  • About 5 minutes — works on any phone, tablet or computer

How digital signing works

  1. 1. You fill in & sign

    Complete the form and sign on-screen with your finger, stylus or mouse.

  2. 2. Instructor countersigns

    Your instructor verifies your licence and adds their signature.

  3. 3. Copies are emailed

    The final signed PDF is emailed to you and filed with Alberta Transportation.

Your information is encrypted in transit, stored in private cloud storage, and shared only with your instructor and Alberta Transportation as required by law.
